Physicals & Re-Certification

All students participating in interscholastic sports (at any level) are required to complete the P.I.A.A. CIPPE/sports physical form and Sudden Cardiac Arrest waiver along with a sports physical.  Both must be completed on or after June 1 of the school year in which the student will be participating in a sport.  The Academy provides access to at least one free-of-charge sports physical opportunity per academic year, although students are welcome to have their own physician complete the form.  Sports for which a physical is not required include:  Youth Swimming and Middle School Tennis clinics.   Please contact the Athletics department with questions.

 

2017-18 Sports Physicals

Harrisburg Academy is hosting free sports physicals on Monday, Aug. 14, 5-7 p.m.  The exams will be administered by OSS Health, our athletic trainer from NovaCare, Lindsey Ciccocioppo, and our school nurse.  Student-athletes participating in all Academy sponsored sports and PIAA Cooperative Sponsorship Agreement sports are required to have a sports physical.  Click for more information.

 

Transfer Students and International Students

Transfer students and and international students interested in participating in PIAA-sanctioned Academy athletics are required to complete the PIAA Transfer Student Waiver Request Form or PIAA International Student Eligibility Agreement, respectively.  Please contact the Athletics Department with questions.

 

Injuries

Students who sustain an injury that requires outside medical attention must complete Sports Physical Recertification paperwork in order to return to the Academy's interscholastic athletics program.  Please contact the Athletics department with questions.
